Simply download the card above (right click and choose "save as...").
For each BoomersGirl Design product purchased, record the name of the item, store where purchased, order number and date of purchase in one of the squares. When all 10 squares are filled, e-mail the card to me at boomersgang at myactv dot net (spelled out to prevent spam). Be sure to include your name, your e-mail address and the name of the FREE product you would like. I will send you a unique coupon code so you can claim your prize!
(Instead of using the card, you can instead just send me a list of the required information.)
